FOCUS ISSUE: STRUCTURAL HEART DISEASE Transcatheter Aortic Valve Implantation State-of-the-Art Paper Stroke Associated With Surgical and Transcatheter Treatment of Aortic Stenosis A Comprehensive Review

چکیده
Stroke is a potential major complication of aortic valve replacement (AVR), transcatheter aortic valve replacement (TAVR), and balloon aortic valvuloplasty (BAV). Although its occurrence is rare, stroke significantly affects survival and quality of life. Based on recent randomized data from the PARTNER (Placement of Aortic Transcatheter Valves) trial, TAVR has recently emerged as the preferred therapy for inoperable patients and as an alternative to surgical AVR in high-risk patients (1,2). Importantly, increased neurologic events associated with TAVR have raised concerns (2). We reviewed the available literature to better understand the frequency, predictors, and clinical impact of post-procedure stroke during different aortic valve therapies for aortic stenosis. Transcatheter Aortic Valve Replacement: A Review Article
Transcatheter aortic valve replacement (TAVR) is a novel therapeutic intervention for the replacement of severely stenotic aortic valves in high-risk patients for standard surgical procedures. Since the initial PARTNER trial results, use of TAVR has been on the rise each year. New delivery methods and different valves have been developed and modified in order to promote the minimally invasive p...

Transcatheter aortic valve implantation was developed to offer a therapeutic solution to patients with severe symptomatic aortic stenosis who are not candidates for conventional aortic valve replacement. The improvement in transcatheter aortic valve implantation outcomes is still of concern in the areas of stroke, vascular injury, heart block, paravalvular regurgitation and valve durability. Co...
متن کاملTranscatheter aortic valve implantation: the new option for high-risk patients with aortic stenosis.
Calcific aortic valve stenosis is the most common valvular heart disease in developed countries. Without surgery, the prognosis is extremely dismal. Therefore there is general agreement that surgical aortic valve replacement should be offered to patients with symptomatic severe aortic valve stenosis. However, surgery is denied to approx. 30-40% of elderly patients with severe, symptomatic aorti...

Current evidence based on more than 8000 high-risk patients with severe aortic stenosis has demonstrated that transcatheter aortic valve implantation (TAVI) is a feasible alternative to surgical aortic valve replacement in selected patients. Despite current promising results on hemodynamic and clinical improvements, there are several unresolved safety issues, such as the frequency of vascular c...

BACKGROUND Transcatheter aortic bioprosthesis implantation is a new treatment modality for patients with aortic stenosis who are inoperable or at high surgical risk. OBJECTIVE To report the three-year experience with transcatheter CoreValve® bioprosthesis implantation. METHODS From January 2008 to January 2011, 35 patients with aortic stenosis (33) or aortic valve bioprosthesis dysfunction ...
